1. Executive Summary
Brazil is the largest pharmaceutical market in Latin America and one of the most dynamic consumer-health markets worldwide. Pharmacy retail sales reached about BRL 246 billion in 2025 (+11.3%) and kept growing by more than 10% in the twelve months to June 2026. Medicines sold without prescription (in Brazil: Medicamentos Isentos de Prescrição, MIP) account for about BRL 34 billion in pharmacies, while the wider consumer-health segment – including supplements, dermocosmetics and personal care sold through health channels – totalled about BRL 52 billion in 2025. The market is attractive but strictly regulated. Every OTC medicine, food supplement, cosmetic or medical device must be regularised with the health authority ANVISA by a Brazilian company that holds the required operating licence (AFE). A foreign manufacturer therefore always needs a local partner – an importer, a licensing partner, a regulatory holder (“detentor do registro”) or its own subsidiary. Choosing this partner is the single most important decision for market entry. Key findings for foreign suppliers: • Fastest access routes are food supplements and cosmetics/dermocosmetics (notification procedures) and low-risk medical devices (Class I/II notification). OTC medicines need full registration, GMP certification and price approval and therefore take longer. • Brazilian companies dominate: Hypera, EMS and Cimed are among the five largest consumer-health players, and Brazilian companies hold 58% of value and 76% of units. Many of them actively in-license foreign products (e.g. Eurofarma generates more than 10% of its revenue from partnerships and licences). • Pharmacy chains and buying networks concentrate demand: the 29 Abrafarma chains generate roughly half of pharmacy sales; the Febrafar federation bundles more than 20,000 independent and network pharmacies. • New channels open up: since Law 15.357 of 23 March 2026 supermarkets may operate segregated pharmacies inside their stores – a new opportunity for both branded and private-label consumer-health products. • EU suppliers benefit from the EU–Mercosur Interim Trade Agreement, provisionally applied since 1 May 2026 (gradual tariff reductions). Regulatory requirements, however, remain identical for all origins; EU-based approvals and GMP inspections can be used to speed up procedures via ANVISA’s reliance mechanisms. • Best product opportunities: respiratory/cold & flu, analgesics, digestive health, women’s health, creatine and protein supplements, vitamins and minerals, dermocosmetics and sun care, lip and oral care, and home-use medical devices.
