1. Executive Summary
Spain is one of the larger and more dynamic self-care markets in Europe. The consumer health market sold through Spanish community pharmacies (non-prescription medicines, self-care medical devices, food supplements, dermocosmetics, personal care and nutrition products) closed 2025 at more than EUR 7.6 billion at retail prices, growing by 3.7% and representing around 30% of the total pharmacy market. In the twelve months to March 2026 growth accelerated to 4.7%, with non-prescription medicines leading the trend. The total Spanish retail pharmacy market reached roughly EUR 26.9 billion in 2025 (+6.1%). On top of the pharmacy channel, significant consumer health volumes are sold through supermarkets and hypermarkets (food supplements, personal care, oral care, first aid), perfumery and drugstore chains, herbalists and organic shops (herbolarios, dietéticas) and a fast-growing online channel. These channels are not included in the pharmacy figures above, which means the total addressable market for non-medicine consumer health products is considerably larger. The market is attractive for foreign suppliers because it combines steady demand growth, an ageing population, a strong pharmacy culture, very high tourist inflows (sun care, travel health, digestive and cold products) and a well-organised wholesale structure that can deliver a new product to almost all of Spain’s roughly 22,000 pharmacies within hours. At the same time, the market is demanding: products must be fully compliant with EU and Spanish rules, labelled in Spanish, competitively priced and supported with pharmacy-level sales and marketing activity. Key conclusions for foreign suppliers: • Non-medicine categories offer the fastest entry. Food supplements, cosmetics/dermocosmetics and CE-marked self-care medical devices can be launched in months; OTC medicines require a Spanish (or EU) marketing authorisation and are best brought in through a licensing partner. • The pharmacy channel is the gatekeeper for credibility. Products that succeed in pharmacy can later expand into parapharmacy, online and grocery. Pharmacy wholesale is highly concentrated: the four largest full-line distributors (Cofares, Bidafarma, Hefame, Alliance Healthcare) together hold more than 70% of the market. • Private label is a real opportunity. Wholesaler brands (e.g. Cofares’ Farline and Aposán, Alliance’s Alvita), retailer brands (e.g. Mercadona’s Deliplus) and online pharmacy own brands are growing and source from qualified contract manufacturers, including foreign ones. • Distribution partners are the most common route for branded products. Spanish consumer health laboratories and specialist distributors regularly in-license foreign brands that are already successful in their home markets and provide the regulatory, marketing and pharmacy field-force infrastructure. • Market access differs by origin. EU suppliers benefit from free movement of goods and harmonised rules; suppliers from non-EU Europe (UK, Switzerland, Turkey, EEA) and from outside Europe need an EU-based responsible entity (importer, Responsible Person, Authorised Representative or Marketing Authorisation Holder) and must handle customs and, for medicines, EU batch release. Chapter 9 provides a structured directory of Spanish importers and distribution partners – full-line pharmaceutical wholesalers, consumer health laboratories that in-license brands, natural-health and parapharmacy distributors, drugstore and perfumery chains, grocery purchasing organisations, full-range FMCG importers and online platforms – together with the best available contact routes for a first approach.
