1. Executive Summary
Italy is one of the most attractive consumer health markets in Europe. Its strength lies less in classic non-prescription medicines, whose market is mature and stable, than in the surrounding "self-care" universe of food supplements, substance-based medical devices and dermocosmetics, all of which are sold primarily through Italy's dense network of around 20,000 pharmacies. Italy is the largest food supplement market in Europe, ahead of Germany and France, and Italian consumers are used to buying health products on the advice of a pharmacist. Key facts at a glance: • Non-prescription medicines (OTC and SOP) reached sales of about EUR 3.2 billion in 2025 (+2.8% in value, 292 million packs, -1.5% in volume). • Food supplements: a market of about EUR 5 billion in 2025, with an estimate of EUR 5.4 billion for 2026; Italy accounts for roughly 26% of European supplement sales. • Including self-care medical devices and pharmacy dermocosmetics, the total consumer health market is estimated at well above EUR 10 billion at retail prices. • The pharmacy remains the dominant channel (about 90% of non-prescription medicine value); online sales are small but growing at double-digit rates. • The trade structure is consolidating fast: wholesaler mergers (QFarma, created from CEF and UNICO in 2025), the rise of pharmacy chains (Lafarmacia., BENU, Dr. Max) and expanding drugstore chains (Acqua & Sapone, Tigotà, Caddy's, dm). • Best opportunities for new foreign suppliers: clinically substantiated niche supplements, probiotics, substance-based medical devices, women's health, healthy ageing/"longevity", dermocosmetics and private label for pharmacy chains, drugstores and supermarket buying groups. • Market access depends on product classification: OTC medicines require a marketing authorisation from AIFA; food supplements must be notified to the Ministry of Health (EUR 160.20 per label) before sale; medical devices need CE marking under the MDR; cosmetics need CPNP notification. The most practical route for most foreign suppliers is a partnership with an Italian licensing/marketing partner or a specialised pharmacy distributor with its own field force, combined with listings at the major full-line wholesalers. Suppliers with private label capability should approach pharmacy chains, drugstore chains and supermarket buying groups directly. Chapter 9 contains a structured directory of relevant importers, distributors and buyers.
